Pre-Arranged Plans are available in the forms of the following:
Cash
Monthly Payments
Estate The Purchaser hereby agrees that this contract will be paid in full by estate or personal funds at the time of death of the Funeral Beneficial.
Insurance
The Purchaser has applied for a life insurance policy on the life of the Funeral Beneficiary, proceeds of which will be used to pay the amounts due under this contract at the time of the death of the Funeral Beneficiary.
Revocable Trust
The purchaser hereby agrees to pay the Funeral Firm the above amounts to be deposited in a revocable trust account as provided by law and Trust Provisions on the reverse hereof.
Irrevocable Trust (For State and Federal Entitlement Programs Only)
The Purchaser hereby agrees to pay the Funeral Firm the above amounts to be deposited in an irrevocable trust account as provided by law (per KRS – 367.937) and Trust Provisions on the reverse hereof. This form must be maintained in the files of the funeral home.
